State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA — The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (eg, dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to [email protected].
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Cowlitz County
In July 2022 the secretary of health suspended the substance use disorder professional trainee license of Kendra Dawn Piper (CO60986555). Piper failed to respond to charges related to her August 2021 convictions in Cowlitz County Superior Court, which include possession of stolen property, forgery, and taking a motor vehicle without the owner’s permission.
Island County
In July 2022 the secretary of health suspended the certified nursing assistant license of Jack Parker Stewart III (NC60509469). Stewart did not respond to charges related to his unsafe treatment of a patient, transferring her from a wheelchair to a bed without a lift contrary to the care plan, which resulted in injury to the patient.
Pierce County
In August 2022 the secretary of health suspended the registered nurse license of Awdrey Paul Palaypay Ng (RN60258507). Ng did not respond to charges related to his 2020 conviction of child molestation and communication with a minor for immoral purposes in Pierce County Superior Court.
Snohomish County
In July 2022 the secretary of health suspended the emergency medical technician license of Tyson King Bushnell (ES01168097). Bushnell failed to respond to an order requiring a mental health evaluation.
In August 2022 the secretary of health suspended the certified nursing assistant license of Margaret Arlene Chesney (NC10052514). Chesney did not respond to charges that she modified and forged letters from the Department of Health about her credential.
In August 2022 the secretary of health suspended for at least 60 months the home care aide credential of Renner P. Santonil (HM60826950). Santonil did not respond to charges that he kissed and touched a resident’s breast where he worked.
Spokane County
In August 2022 the secretary of health suspended the registered nursing assistant license of Beverly Ann Moore (NA00200586). Moore did not respond to charges related to her taking medication, a Symbicort inhaler, from a patient and reporting it missing.
Whatcom County
In July 2022 the secretary of health suspended the certified nursing assistant license of Amanda Kate Lyons (NC60386319). Lyons did not respond to an investigation into a complaint alleging she’d taken medication from residents where she worked.
