State revokes, suspends licenses, certifications, and registrations of health care providers

OLYMPIA – The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following healthcare providers in our state. The department also immediately suspended the notarization of anyone banned from practicing in other states.

The Quality Assurance Department for Health Systems works with committees, commissions and advisory boards to define approval standards for more than 80 health professions (e.g. dentists, nurses, consultants). Information on health care providers can be found on the agency’s website. In the How Can I? Section, click Find a Healthcare Provider License. Section of the Ministry of Health homepage (doh.wa.gov). The site contains information about a health care provider’s license status, ID expiration and renewal dates, disciplinary measures, and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who believe a health care provider has acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In November 2021, the Ministry of Health suspended the license to qualify as a qualified nurse and a state-recognized nurse Brooke McKenzie Walton (NC10096108, NA60858291) for two years for unprofessional behavior and violation of practice standards. Walton accepted money and items from one patient, stole money from another patient, and made unauthorized withdrawals from another patient’s bank account. In December 2020, she was charged with identity theft in the Benton County Superior Court.

Clark County

In November 2021, the Nursing Commission suspended the license to practice as a nurse Roxanne Dawn Kokkeler (RN60401543). In June 2021, Kokkeler’s license was revoked by the Oregon State Board of Nursing for practicing without a current license and for not cooperating with the board during an investigation.

King County

In November 2021, the nursing assistant program suspended the registered nursing assistant license Feven Tekeste Matewos (NA60181969) pending further action. The Ministry of Social and Health Services found that she was neglecting a vulnerable adult under her care.

Circle of Kittitas

In November 2021, the Ministry of Health rejected an application to reactivate the qualified nursing assistant Kristi Jo Johnson (NC10044126). Johnson was convicted of reckless driving in 2013 and drinking under the influence in 2015 and 2016.

Mason County

In November 2021, the Nursing Commission suspended the license to practice as a nurse Rosamond L. Hoover (RN00173669) for failing to follow a substance use monitoring program.

Pierce County

In November 2021, the Chiropractic Commission obtained the X-ray technician license from. revoked for good Alayna S. Thompkins (CX60486392), without the right to reapply. Thompkins was found guilty of theft and trafficking in stolen property by the Pierce County Superior Court.

Spokane district

In November 2021, the Nursing Commission suspended admission as a nurse for an indefinite period Christie Jeanette Bates (RN60753642). Bates derived controlled substances from her workplace.

In November 2021, the Home Care Aide Program suspended the Home Care Aide license Rachel Hanna Kintner (HM60701510) for five years after being fired for having a romantic and sexual relationship with a patient.

Not in the country

Nevada: In November 2021, the Nursing Commission suspended the nurse license indefinitely Jacqueline K. Smith (RN00138985). Smith failed to complete an approved nursing refresher course.
