State revokes, suspends licenses, certifications, and registrations of health care providers

OLYMPIA – The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following healthcare providers in our state. The department also immediately suspended the notarization of anyone banned from practicing in other states.

The Quality Assurance Department for Health Systems works with committees, commissions and advisory boards to define approval standards for more than 80 health professions (e.g. dentists, nurses, consultants). For information on disciplinary action against doctors and nurses, see the Washington Medical Commission (WMC) website. Questions about WMC disciplinary action can be sent to [email protected].

Information on health care providers can be found on the agency’s website. In the How Can I? Section, click Find a Healthcare Provider License. Section of the Ministry of Health homepage (doh.wa.gov). The site contains information about a health care provider’s license status, ID expiration and renewal dates, disciplinary measures, and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who believe a health care provider has acted unprofessionally are also encouraged to call and report their complaint.

king district

In October 2021, the pharmacy commission suspended admission to become a pharmacist for an indefinite period Jessica Ta (IR60684519). Ta failed to comply with an order that required her to undergo a mental examination after she was charged with unprofessional behavior in June.

In October 2021, the Nursing Assistant Program and the Counselor Program suspended the certified nursing assistant license Santana Wangui Cubi (NC60865627) for at least five years and was fined $ 5,000. Cubi was charged by the Pierce County Superior Court with second degree criminal ill-treatment that resulted in the death of a 95-year-old patient.

Skagit district

In October 2021, the Nursing Commission suspended the license to practice medicine and the license to practice medicine as a state-approved nurse Robyn Margot Choffel (AP30003913, RN00103955) for at least 12 months. Choffel pleaded guilty to attempted criminal impersonation in the Skagit County Superior Court after she and her ex-boyfriend prescribed a controlled substance in violation of a court order prohibiting her from prescribing drugs to her family or partner .

In October 2021, the Chiropractic Commission obtained the chiropractic license from. suspended indefinitely Steven M. Rindal (CH00000920) for lack of cooperation in a DOH investigation.

Spokane County

In October 2021, the Ministry of Health received the license to practice as a qualified nursing assistant Alexander Gregory Baacke (NC60611406) for five years. Baacke was convicted of theft and identity theft in Spokane County Superior Court.

Not in the country

Colorado: In September 2021, the Nursing Commission rejected the application to practice as a nurse Nora Ager West (RN61058220). West’s nursing license was revoked in August by the Colorado State Board of Nursing for violating the Nurse and Nurse Aide Practice Act.

Ohio: In October 2021, the Nursing Commission suspended and billed the registered nurse’s license with immediate effect Rhonda Joy Phillips (RN60105348). Charges allege Phillips was suspended from the Ohio Board of Nursing in May 2021 after pleading guilty to two counts of misappropriation to procure dangerous drugs. Phillips must respond to the charges to request a hearing.
