State disciplines of health care providers

OLYMPIA – The Washington State Department of Health has disciplined or withdrawn charges against the following health care providers in our state.

The Quality Assurance Department for Health Systems works with committees, commissions and advisory boards to define approval standards for more than 80 health professions (e.g. dentists, nurses, consultants). For information on disciplinary action against doctors and nurses, see the Washington Medical Commission (WMC) website. Questions about WMC disciplinary action can be sent to [email protected].

Information on health care providers can be found on the agency’s website. In the How Can I? Section, click Find a Healthcare Provider License. Section of the Ministry of Health website (doh.wa.gov). The site contains information about a health care provider’s license status, ID expiration and renewal dates, disciplinary measures, and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who believe a healthcare provider has acted unprofessionally are encouraged to call and report their complaint.

Clark County

In October 2021, the Ministry of Health issued an end to the supervision and monitoring of the agency-affiliated advisor Ryan Leroy Poland (CG61052931).

Grays Harbor County

In September 2021, the Ministry of Health denied the application to work as a home care worker Amy Lu Thomas (HM61113985). Thomas was previously fired from work in an adult home because he was compromised at work by illegal drugs.

King County

In August 2021, the Unlicensed Practice Program issued a letter of intent to issue an injunction Johnny T. Stine for the unauthorized medical practice. Stine is said to have given patients an unapproved COVID vaccine that he developed despite not having medical approval.

In September 2021, the Ministry of Health issued an injunction Hai Deng for illicit massage therapist and fined $ 1,000. Deng offered massage services while employed as an unlicensed massage therapist.

In September 2021, the Ministry of Health issued an injunction Yali Guo for illicit massage therapist and fined $ 1,000. Gou offered massage services while employed as an unlicensed massage therapist.

In October 2021, the Unlicensed Practice Program issued a letter of intent to issue an injunction Nadia Umarov for unauthorized nursing. Umarov allegedly misrepresented herself on her Instagram page by claiming she was a licensed beauty injector and registered nurse, even though she never had a registered nursing license.

In October 2021, the Unlicensed Practice Program signed an agreement Dr. Kristine Brecht and Aesthetic Rejuvenation and Spa by Dr. Brecht. Brecht agreed to cease operating an outpatient surgical facility permanently and pay a fine of $ 20,000 within two years.

In October 2021, the Dental Commission reached an agreement with the dentist Lance B. Timmerman (DE00008229). Timmerman must comply with terms and conditions, including unannounced audits, for four years, pay a $ 20,000 fine and $ 8,000 expense to the commission, complete 29 hours of training within 12 months, and all misleading and inaccurate information remove from his website.

In October 2021, the Nursing Commission appointed a nurse Jessica Kathryn Whitehead (RN60574680) with unprofessional behavior for failing to follow a substance use monitoring program.

In October 2021, the dental commission granted the reinstatement of the license to practice medicine as a dental assistant Shamay Monique Haskin (D160058955).

In October 2021, the Dental Commission reached an agreement with the dentist Prabhjot S. Sidhu (DE00010707). Sidhu must comply with the terms, including unannounced audits for three years, retention of a practice monitor for one year, or completion of an assessment and rehabilitation program, reimbursement of four patient expenses, and reimbursement of $ 6,000 to the commission for investigation and hearing expenses.

Kitsap County

In October 2021, the Nursing Assistant Program hired a certified nursing assistant Taressa Browning (NC60869297) with unprofessional behavior. Browning was convicted in the Kitsap County Superior Court in prison for extortion, malicious persecution, criminal attempt and double first degree sexual misconduct.

Okanogan County

In October, the Nursing Commission indicted a licensed practical nurse Luke Aaron Richter (LP60702263) with unprofessional behavior for allegedly sleeping at work and providing false information about his patient care.

Pierce County

In October 2021, the nursing assistant program commissioned nursing assistants and home care assistants Joseph Ndungu Njoroge (NA60795700, HM60814101) with unprofessional behavior. Njoroge allegedly misrepresented himself as a certified trainer, handing fake certificates of home care to at least 10 people he raised money from.

In October 2021, the Ministry of Health ended the probation of the drug disorder intern Renee Michelle Cline (CO60971093).

In October 2021, the Nursing Commission concluded an agreement with the nurse Elizabeth Tracy Anderson (RN60136609) requesting participation in a substance use monitoring program.

In October 2021, the Dental Commission withdrew the charges against the dental assistant Karla Garcia (D160689122).

Snohomish district

In September 2021, the Ministry of Health revoked approval as a mental health advisor Felix Reynold Neals (MC60485434) and put its application for approval in pending status. The department found that Neals’ application did not provide sufficient evidence to complete a required degree.

In October 2021, the Nursing Assistant Program booked a registered nursing assistant Melvina Boachie Danquah (NA60294127) with unprofessional behavior for allegedly accepting $ 14,000 in payments from a patient and failing to return the funds.

Spokane County

In October 2021, the Nursing Commission decided to lift the supervision and monitoring for licensed practical nurses Kimberly Ann Allen (LP60308056).

In October 2021, the Nursing Assistant Program hired a certified nursing assistant Jeffrey Kenneth Marks (NC60475255) with unprofessional behavior. Marks was convicted of second degree rape of a child in Pend Oreille County District Court.

In October 2021, the substance use disorder programs, agency-affiliated counselors, and nursing assistant programs entered into an agreement with addiction professionals, agency-affiliated counselors and certified nursing assistants Larry Gene Howard (CG60854071, CP60506174, NA60852767). Howard’s license as an affiliate consultant has been suspended for four years and is fined $ 1,000 and completed 8 hours of training in six months. Howard’s license as a Registered Nursing Assistant is suspended indefinitely unless he meets the conditions for reinstatement.

Thurston County

In October 2021, the mental health counselor program changed an agreed arrangement with a mental health counselor Becky M. ` (LH00008881) allows ` an additional six months to complete the training requirements of the previous assignment.

In October 2021, the mental health advisor program hired a mental health advisor Cassie A. Icenogle (LH00010368) with unprofessional behavior and failure to report. Icenogle allegedly misdiagnosed an underage client who later attempted suicide. Icenogle also reportedly failed to notify child protection services of the customer’s allegations of father harassment and abuse.

In October 2021, the Ministry of Health restored the license for the substance use disorder intern Bernadette Lacey Cruz (CO60324514).

Yakima district

In October 2021, the Ministry of Health restored the license to practice medicine dependent on chemicals Amanda Rose Rodriguez (CO60819675) after completing the terms of a substance use monitoring program.

Not in the country

Alabama: In October 2021, the Osteopathic Medicine and Surgery Board reached an agreement with an osteopathic doctor and surgeon Vinson M. DiSanto (OP00001800), where DiSanto’s license suspension was lifted and he was placed on probation for two years. DiSanto agreed to pay a $ 2,000 fine and subject to unannounced audit visits during his probationary period.

Delaware: In October 2021, the Nursing Commission appointed a nurse Nathaniel David Swift-Erslev (RN60832241) with unprofessional behavior. Swift-Erslev’s license was suspended for three years by the Delaware Board of Nursing on alleged diversion and substance use in June 2021.

Florida: In October 2021, the Nursing Commission approved the end of the supervision and monitoring of the nurse William Bolling Brawley (RN60397643).

Note to editors: Healthcare providers charged with unprofessional conduct have 20 days to reply in writing to the Department of Health. The case then enters the billing process. If no disciplinary agreement can be reached, the case will be heard.
